Capture the claim, source, reporter, possible harm, and urgency without collecting protected information.
Warn or suppress when needed
Continued reliance on a possibly wrong high-risk instruction triggers immediate review and may remove the item from ready-to-use surfaces.
Investigate dependencies
Find every resource, calculator, search result, and local recommendation that uses the affected claim.
Publish the reviewed correction
Preserve the prior version, record the reviewer and decision, update dates and sources, and explain the material change.
