Record what you currently believe, what decision depends on it, what evidence could change it, who owns the follow-up, and when you will revisit it. Never blend assumptions with verified facts.

Every early plan contains assumptions. The risk begins when an assumption loses its label and later choices treat it as settled.

If you are new to ownership

Capture statements that begin with probably, usually, I think, or the vendor said. You are not expected to know everything; you are expected to keep uncertainty visible.

If you already run a practice

Use the register to find old beliefs that still shape budgets, staffing, software, or timelines after the practice changed.
